Access your clinic and hospital records for visits prior to November 1, 2017 through our legacy systems. Your records from clinic and outpatient visits are housed in HealthLink and your records from a hospital stay or other hospital-based testing will be found in My Hospital Record, by Relay Health. Medical records for visits beginning November 1, 2017 may be accessed on MyChart.
